Should the wording be “declined medical treatment” rather than “refused”? It feels very different.
“Refusal” is pretty standard in a medical context; it is the opposite of “consenting” to medical assistance. It implies that the individual made an informed decision to not seek medical attention, not the much more passive “decline.”
